What documents do I need to provide for my first appointment?

• Please bring your driver’s license or picture identification,

• ​we may ask you to also bring your most current insurance card 

• We also ask that you download our New Patient Forms, print them out, complete them and bring them with you to your appointment. 

• If relevant, also bring in a list of the medications you currently take.

How early do I need to be for my appointment?

If you are a new patient, and you haven’t completed the new-patient paperwork, please arrive 15 minutes early to complete it. If you have printed out your new-patient paperwork from this website and have it completed, or if you’re a returning patient, please arrive 5 minutes prior to your appointment.

How often will I need to be seen?

How frequently you need to return to Vista Rejuvenation & Wellness Center depends entirely on the details of Therapy Plan. Some Therapies require only an initial office visit while other Therapies require weekly visits.  As an example for sexual dysfunction therapies, only one Doctors Office visit is required.

For most Harmone Replacement Therapies, weekly visits are needed to correct and maintain adequate hormone levels for both men and women.  These visits are usually very quick – In and Out taking about 10 minutes.

Our weight loss Therapy generally requires a initial office visit and then monthly maintenance visits, frequency of visits depends on the particulars of your program, which is tailored to your needs.

IV THERAPY

Is IV therapy a safe treatment?

IV Therapy is a safe treatment to take as it is FDA approved and proper evaluation is undertaken before administering it. IV treatment is a method of delivering vitamins and minerals to people who are deficient in them. In the clinical environment, it is a normal practice that is quite standardized and safe.

How long does it take?

It takes 30-45 mins for the IV to complete. It depends on your tolerance for receiving an IV. Some people can have an IV run in 20 minutes, other people it can take 60 minutes or more. Our helpful team will assist you in determining the best speed for you.

Are there any side effects?

There are no considerable side effects to it, however, you may experience slight discomfort. To avoid that discomfort, inform your doctor about any drugs or supplements you are taking and any previous reactions. Our team will keep an eye on your responses.

Our Goal is to reduce the risks associated with the typical approaches to hormone therapy. Testosterone (men) is typically injected.
